Legal Question in Disability Law in California

I am receiving workmens comp in CA. I will be moving to CO to be with famly as I now have terminal cancer too. I am still under the care of a WC Dr. in CA. How can I transfer my care to CO and how do I find a WC Dr. out there to treat me?
