declaration of non taxable income

Filed bankruptcy pro se. Attended meeting of creditors & was told to provide income tax return. Indicated that I do not file because my income is non taxable social security disablity. Was told to send a declaration. What does this consist of:form, letter or other legal document?

Re: declaration of non taxable income

At the beginning, state: "I, [your name] declare or affirm as follows:" Then, number each subsequent paragraph. Recite the facts of the years you have not filed tax returns, and the reason why. You might also state the amount of SS income you received each year. At the end you state, "I declare under penalty of perjury that the foregoing is true and correct. Executed this [fill in date] day of [fill in month] 2007 at [fill in city], California." Then on a few lines down, sign and print your name below your signature. Send it to whomever asked for it, and the U.S. Trustee's office, as well. Make sure to put your BK case number on it.
