Legal Question in Immigration Law in California

I am a 25 year old going in for a US citizenship test in a few weeks and am gathering documents for the interview. The Interview Document Checklist states:
- If you have ever been arrested or detained by any law enforcement officer for any reason and no charges were filed, bring an official, certified statement from the arresting agency or applicable court indicating that no charges were filed.
- If you have ever been arrested or detained by any law enforcer and charges were filed for any reason, bring an original or certified copy of the arrest record and complete court disposition for each order.
I've had three incidents that I am not sure if I should mention/do not have the documents for:
1. Petty theft at age of 15. Since my records were sealed, do I still mention it? If so, how do I get documentation?
2. Misdemeanor for Minor in Possession of Alcohol at age 20: Charges were dismissed and I took a Drug and Alcohol Class, but I do not have any of the documentation. Where do I obtain it?
3. Revoked license for driving under the influence (.07) at age 20: Charges were never filed and I never went to criminal court for it. Went to court with the DMV and the DMV revoked my license for a year, but the court never contacted me for an arraignment date. I have no idea how this happened, but it's been 5 years since this time and I've had my license back for nearly 4 years. I don't think it's on my record. Is this worth mentioning?
