what kind of form do i need to fill out demanding former employer to pay or further action will be taken without notice

There is form and there is no legal requirement that you provide notice before suing. If you are owed wages, etc., call an employment law attorney to discuss. Many of us offer a free initial phone consultation. Also, wage laws usually provide that your employer pay your attorney to recover wages which are willfully withheld.
