my lawyer had made many mistakes on my case and taking the full 40% including providing medi-care with a wrong date then charging me for hours he spent fixing his mistake what can I do?

You could probably take him to fee-dispute arbitration, especially over the additional hourly fee he charged. Normally that's not allowed on contingent-fee cases. Contact your local bar association.
