Legal Question in Wills and Trusts in California

I'm in California and my father passed away in 2012. At that time, we were joint tenants of his residence. In his will, he stated that I was to receive his property. He did not fully exclude my brother so he filed a renunciation of inheritance and

I had my 50% of the estate assessed and it was not over $150, 000. I also petitioned the probate court for succession to real property which was granted.

Unbeknownst to me, the deed was never transferred to my name and the succession was never recorded. I'm not sure what form I need to submit to the assessor's office to get the deed transferred to my name.

Affidavit of Death of Joint Tenant or an executor's deed transfer?

Please help, I had an attorney assisting me at the time but I was failed by two separate firms on the proper steps to complete this.
