Distributing a living trust

My mother passed away and I am told that I must file a Notice of Trust Becoming Irrevocable. New CA

law, however I cannot find any sample forms of this Notice and understand it should be served on beneficiaries prior to sending a copy of the living trust to named beneficiaries.

Check the Probate Code Section 16061.7 for what is required. There is no actual form, per se, but the cde section will tell you what you need to do.
