What is entailed when transfering a revocable trust from one trustee to another- completely changing the original trustees?

It sounds like you want to either substitute a trustee or put in place a successor trustee. If the trust is still revocable it should be an easy amendment to the trust. However, proper formalities need to be followed.

Depending on the terms of the trust, this may or may not be an allowable change. Is the original settlor still alive?

If it is allowed, then I would suggest making an Amendment to trust wherein you cite the authority to change trustees as well as cite the old and the replacement language in the trust.
