Carpotunnel Syndrome

I been on treatment due to carpotunnel syndrome and a syst that was removed it from my left hand, on 03/09 my Dr. sent to my employer my diagnosis as a permanent disability now my employer sent me 3 letters that I need to sign and returned to my employer but I don't want to sign something that I really don't understand. Please advise me. Thank you.
