Colorado Name Change - How To

I would like to change my last name, here in the satae of Colorado. Please tell me where to begin. All of the documentation I have founf says submit this form with the LEGAL DOCUMENT effecting the change. I don;t know how to aquire that document.

The change does not involve a marriage or divorce.

One must petition the court for a change of name. If you can't get this far, how will you get the rest of the way?

You must petition the court. The forms and instructions may be obtained online at www.courts.state.co.us. Once you have the forms completed, take them to the County Court rather than the District court. The judge will want to know if you're doing this to defraud anyone, such as avoiding creditors. S/he'll give you instructions from there.
