Florida Fact Information sheet asks for children's names and addresses, do I have to provide this information?

Yes. One purpose of the question is to help determine whether you are financially responsible for anyone other than yourself. The questions should be answered to the best of your ability, and truthfully. I typically recommend to my clients they not include their complete Social Security Number or bank account numbers, and only include the last 4 digits of each. Usually the attorney for the creditor does not object to this.

If you have received a judgment, which it appears you have, you should consult with an attorney experienced in debt relief and asset protection. There are many protections from collection activity that most people do not know about, and there are many dangers that exist for people who try to do this on their own. You may have options that would prevent collection of the judgment.
