What is the easiest and most effective way to add a spouse to a house title? Quitclaim deed transferring the property from one spouse to both spouses as joint owners with right of survivorship?

I would suggest that you contact a real estate attorney in your area. There are several options. The cost of paying someone to do it correctly for you the first time will be much less than the cost of fixing errors when they are discovered in the future. I rarely see DIY deeds prepared correctly. Some of errors are easily fixed, and some actually cannot be fixed.

I would certainly see an attorney for several reasons. First, there is a question of preserving your title insurance coverage. Second, if this is your home, you can be advised about other options for the form of ownership. Third, we often come across deeds in title examination that were done by individuals and find that something incorrect creates a title issue. Finally, there may be some reasons that it would be better to keep the title individually held with an estate plan used to protect the spouse.
