You need to review the copy of the 2014 return so that you are comfortable that it is accurate, and then sign the return and mail it in. Illinois most probably wasn't filed either. Sign each return with a current date and mail them to IRS and to Illinois Department of Revenue by Certified Mail, return receipt requested so that you can obtain proof of filing.

If you are not confident that your accountant prepared the return accurately, take it to another tax professional for review and possible reconfiguration of the return before filing.
