Non response to heath coverage mailing leading to removal of dependant care. Note from employer:
If you have dependants on your insurance and you receive a letter, you mustsubmit the requested information to the address or fax number in the letter. If you fail to submit the necessary information, your dependants may be removed from the insurance.
