Legal Question in Family Law in Massachusetts

I was never notified by the court for a hearing on to have my child support reduced by my ex husband. I received a Temporary order for the reduction stateing I did not show up. When I called the court, they had an old address that the notice was mailed to,however my order came to the right address... The Judges secretary told me to file a Modification to Vacate the order, How should I word the request?
