biological father giving up rights to child

My child's biological father lives in North Carolina & my daughter & I live in Michigan in which state she was born. Her father & I were never married. He is 9 months behind on child support & has only seen her twice when she was 2 months old, he has had nothing to do with her since. What forms can I get for him to sign off his rights so my new husband could adopt her? What would it cost to fill these forms & for the adoption? What would it mean for her biological father, if he signed off his rights?

The adoption process itself would result in the biological father giving up his rights. If he does not agree, you'll probably not be able to do the adoption. I'd contact him and explain how much support you will seek until the child turns 18 and explain to him that it is in his best interests to allow the adoption. Also, in a proceeding that is as important as this to both you and your child, you should definitely hire a competent attorney. Borrow $ if you have to but use an attorney. I'm not aware of any pre-printed forms for termination of parental rights. You would be best advised to hire an attorney to handle the termination and adoption to make sure that it is done correctly.
