Should I file for sole, legal, physical custody of handicapped daughter-dad hasn

Daughter is 18 months, severely multiply handicapped. Father hasnt contacted us in almost a year. As the custody order stands, we need to consult with each other for major decisions, and I am facing rather large medical decisions concerning her. I dont feel i should have to contact him to consult with him on an issue he has had nothing to do with, but i dont want him to be able to come back on me for not consulting with him. Can any one help?
