What would I have to do and what would my company have to do to place me on workers compensation for life due to insanity?

Assuming your company has workers' compensation insurance and is not self-insured, it is really not the company's decision. It would be the decision of the insurance carrier. If the insurance carrier does not believe that your insanity is due to conditions that actually existed on the job and that the job conditions significantly aggravated or accelerated the onset of your insanity, they would litigate the issue in the workers' compensation agency. These are typically difficult cases because they rarely are solely due to job conditions. The insurer would look into your case history and dig up everything and anything that would show that you had issues before and that they just re-manifested themselves on the job.
