Legal Question in Family Law in Minnesota

I have plans to move out of state. I have sole physical and legal custody of my child. The visitation is all verbal agreement between parties as decided by a judge. The only contact order is he can call her every day at certain times. He is only allowed to a see her if supervised. Which had happened intil now. My child is almost 12 and her father I found out has always slept in same bed. I am not comfortable with that and neither is my child. She Wants to move with me but he says no. From my understanding he doesn't have a say because a judge only granted him phone contact and supervised visitation that he would need to pay for but doesn't. Is this correct? What should I do to legally cover my child and me from getting in trouble for leaving state? I do not know if she has been abused by him tho because she is very shy and angry when shed come home. His supervised visitation is because of drugs.please help

If your court order states there is parenting time, even as agreed by the parties, you must have consent of the other parent or an order of the court to relocate with children out of State,
