There is a statute that defines when a person shall be charged as a prior offender, defines other such categories, and sets forth various provisions for the enhancement of punishment. In most situations, the initial DWI arrest at least is going to stay on a person's arrest record, and law enforcement may, in fact, treat such a person differently than a person with no record. If there was an administrative suspension or revocation of yoru driver's license, tat would still show up on your record. The full answer to your question might depend upon various factors that you do not include in your question. Where is the new offense? Is it a municipal charge? Did you take an SIS on the original charge 15 years ago? Definitely consult with and hire the best defense attorney you can afford, and you should be O.K.
