Legal Question in Credit and Debt Law in New York

Medical bills

I have only now been billed for a medical procedure carried out over a year ago, and in the meantime I have changed my insurance company. Is the original or the current insurer going to cover this at all and if not, am I liable to pay anything? I would have thought that if the provider doesn't present their bill within a reasonable time frame (say 90 days?), they forfeit their claim. If that is the case, what's the best way to respond?
