what legal form do i use to authrorize a contractor my neighbor hired to gain access to my property so that they can render services to my neighbors house so that my neighbor or the contractor will bre liable for any damages to my property and guarantee complete cleanup of all debris and garbage

I know of no such form.

If your neighbor's contractor damages your property you have legal recourse against either or both for any damages you may have suffered.

Those forms are a summons and complaint, used to begin a lawsuit.

However, before suing for damages, a well written letter from an attorney, demanding full cleanup, may be sufficient to get what you need done.
