This varies widely from court to court. However, many courts have a standard form that you can obtain from the Clerk of Court then complete and file with the Clerk. You can call the Clerk of Courts (ask for the Criminal/Traffic Division) and ask them. There is usually also a filing fee that you must pay and you will normally also be required to attach a copy of your current insurance and a letter from you employer verifying your employment and the hours you work. When you make the request, be sure to include the travel time to and from your place of employment. Before you file you also want to make sure that any "hard time" (time during which you are not eligible for privileges e.g., under a DUI suspension) has passed or that you ask for your privileges to start after the hard time is over or your motion will simply be denied and you will be out your filing fee.
