Giving up custody to grandparents

My daughter has full custody of her 4 year old son. She is a recovering addict. She has been having her father and I take care of the boy. She would like to relinquish full custody to us. The biological father is in agreement. How do we go about doing this? Do we need to hire an attorney or are there forms we can submit to the courts ourselves?

In order to give up custody to the child and both parents agree, it would be my suggestion to file an action in Juvenile Court. It is best done by an attorney who knows what they are doing. I have done this before. You can always choose to do it on your own, but problems may arise in which an attorney may be necessary anyways
