Legal Question in Immigration Law in Ohio
work permit name error
I have received a work permit but it has only two names on it.I would like to add one last name. How do I go about doing this?

If the name you want to add appears on your birth certificate or it is due to a marriage, you can fill out another I765 and mail with either document above, as applicable. If you originally filed for the work permit with your full name and the mistake is on uscis' part, then you just send a letter and no fee. Hope this helps.
