Legal Question in Personal Injury in Ohio

A friend of mine is incarcerated at a county jail. While in supermax he was assulted by another inmate who was not suposed to be out of his cell.He requested medical attention and to this date hasn't had any. The CO's admitted they made a mistake and he was told by one of them to file a complaint. I was told to request forms to file but I am unsure of what type of forms to request and also where to get them. I checked with county courts and they sent me to the state of ohio website. I couln't find forms there for this. I am unsure if this would be considered a class action. Please help.

I would suggest loking in the Court of claims for the State of Ohio.
