moving a case from smal claims to civil court.

What form do I need to move a case from smal claims to civil court. My employer employer did not pay me. I started a small claims. He got it moved to a Civil case. The court said I need fill out a form. No one can tell me what form I need.

I would recommend that you speak with an attorney. If the case has already been moved from the small claims to the civil docket then there is nothing that you need to do other than continue to pursue the claim. However, civil court is more complicated than small claims and your employer will likely be represented. You can contact my office if you would like more information.
