In my divorce agreement my ex is to sign over my car to after i get it paid off can he refuse to sign it if its in the divorce agreement?

Of course he can. That said you could haul him into court. Get an order for him to sign the title to yo9u and if the MSA allows for it he has to pay your ay\ttorney's fees.
