Adding a name to a property deed

what forms and procedures do i need to add my significant other to my property deed on my home

The form is called a deed. You would be the grantor and you and he would be the grantees. You would then have to file the deed with the recorder of deeds.

Be advised you need to take care as to how the new title is worded. Unless you are married you can not be tenants of the entirety you can only be joint tenants.

Given that you will be making a large gift to your significant other you should consult with an attorney.
