assignment of physicians

what is the legal responsibility of a hospital / physician to a patient when an internal medicine physician is assigned and in charge of a head injured patient; when the patient obviously required a neurologists control of his/her care.

the internal medicine physician recorded incorrect information in medical records (diagnosis/prognosis). The incorrect information caused patient to be continuously regarded as such, (vegetative state)and has been refused the care and therapy he needed after discharge from hospital.

If the patient has been denied necessary care due to incorrect information being being entered by the doctor you refer to, the obviously the hospital is liable.
