Legal Question in Landlord & Tenant Law in Tennessee

I purchased and downloaded your form for renting a room out in the house I own. It was the one for furnished rooms. It appears to me that it is not a finished document. There are blue notes one it indicating something was deleted for not deleted or add this and so on. It's very confusing. Maybe I'm looking at wrong. Please look into this and let me know ASAP. thanks.
