You need an SR-22 (proof of financial responsibility) from an insurance company and a form DIC-37 from DPS. I have never heard of a driving record being required for an occupational licenses. You will also need a petition explaining why you need the occupational license and draft copy of the court order. Suggest you hire an attorney for this.

Generally you do not need to provide a copy of your driving record when you are applying for an occupational license in criminal court as the prosecutors have access to the records and can print them themselves. If you are attempting to get an OCDL in civil court, it is different and the judge may require that you provide the record.

When you go online to order the license, you will see different choices. Generally the one that provides the longest term of driving and information is the one the judge wants to see. When they are considering giving an OCDL, they want to know whether you are a safe driver.

You really should use a lawyer to take care of these. I don't even do them myself; I refer them out.
