Is there a way a beneficiary can be illegally changed on a life insurance policy?

Yes.

If the owner of the policy did not sign the beneficiary form, but instead someone else forged the owner's name on a beneficiary form, the change of beneficiary would not be legal.

Also if the owner of the life insurance policy agreed in a different contract - for example a final decree of divorce - to name someone as a beneficiary but failed to do so, there would be legal issues.

Otherwise, the owner of a life insurance policy has free reign to name whomever he or she wants.
