how can i get death certificate of my grand mother.she has died ten year ago.i am resident of delhi.please tell about whole procedure with required documents.

Dear Sir / Madam,
In the event of death, the local doctor / hospital issues a Provisional Death Certificate which is submitted to the concerned crematorium, who after the funeral rites have been completed sends the form with the provisional Death Certificate to the local Municipality. You will have to make an application to the local Municipality giving detail of the deceased's name, residential address, address of death, and exact date, month and year of death along with a copy of the provisional Death Certificate, if you still have it. The Municipality will extract and issue a copy of the Death Certificate to you.
