Legal Question in Family Law in Indonesia

Good afternoon,

I was living here in Philippines and I have an Indonesian fiancee. He is a catholic. Unfortunately, he doesn't have surname on his birth certificate. Since we are getting married soon, I would need to put his surname next to my name as part of our culture here. Could you please tell me what will be the first thing to do for him to add surname? Would it be hard for him to renew his US visa if he changed his name? Thanks for your help.
