hi sir ..my birth name was Rajesh (without surname)...I born on 25th -June- 1984 in kanpur....later at 2010 i changed my name to Raj Kushwaha....i want to apply for Birth Certifiacte Now ..how do i do that and in which name. .

why do you need a birth certificate?? go for the procedure for the change of the name.
