Can health insurance be included in a severence package?

As a Franchise Attorney I can the answer is most likely yes. However, you need to check with the insurance provider as the policy may only cover "employees." They may have a special rider to the existing policy that can be used, or you could always provide the cash equivalent to the departing employee. Consult with a good business or franchise attorney in your area for specific advice.
