Legal Question in Credit and Debt Law in Alabama

medical collection agency fees

I received a statement from a medical debt collector stating that I owed $20 more than the statement from the doctor showed. I was informed it was a ''collection fee.'' Is that legal in the state of Alabama? If not, how do I fight it?
