My mother-in-law has lived with us for several years. After several illnesses she requires around-the-clock care. We feel that she should be in a nursing home.
My brother-in-law holds her POA and refuses to admit her to a nursing home.
Can she be admitted without his approval?

You and your brother need to meet with an Elder law attorney ASAP to discuss your options with regard to Medicaid, VA (perhaps) and other estate planning issues. Tell your brother that the longer he waits, the more it will end up costing him. At $5,000 per month, nursing homes are expensive, so if you can qualify for Medicaid, that is $5,000 each month that you and your brother will save. Tell him that this is no matter to wish away. It needs to be dealt with.
And yes, your mother can be admitted over his POA.
