How do you go about exchanging primary custody

My husband and I live in NJ but his children are with his exwife which she is remarried, they share joint custody, his exwife has been callin the father because the 14 year old has been giving her and his step dad lots of trouble they agreed on sending the 14 year old boy to NJ to live and go to school here, the schools in NJ want a document from the court statin she gives custody to the dad, how does she go about doing this her self without a lawyer? does she file a motion in court?

If the divorce judgment was entered in New Jersey it is simply a matter of filing a motion under that judgment here indicating that the mother and father consent to the revision of the custody arrangement under the judgment of divorce; serving it upon her and submitting the appropriate order to the court for entry under the New Jersey divorce judgment.

If the divorce judgment was entered in another state the same can be done there.
