Am I Entitled To Receive Workers Comp?

i injured my knee at work. the company doctor is not helping and the physical therapy is making it worse. this happened over a month ago now.

since i have been unable to return to work, the company wants me to fill out a leave of absence form. should i?

i am going to pay to see my own doctor and find out what is really going on. since it has been past 30 days, is my employer responsible for this bill?

also, i get paid cash for doing a very simple job, close to home and i do not have to do any walking. i do not have to fill this money on my taxes. will this hurt my chance of any kind of suite i may have?

The company does not have to pay for you to go to your own doctor. They only have to pay for "authorized" treating physicians. You are entitled to ask for a panel of four physicians. If you do, they have to give you the names of four treating physicians from different practice groups. If you do request a panel of four, ask specifically for four orthopaedic surgeons; otherwise, they will give you the names of four doctors, and they may not all be the same type of doctor, i.e.: one orthopaedic surgeon, two neurosurgeons, and one physiatrist. Of course, all of this assumes your employer has workers compensation coverage. If they have 4 employees or less, they don't have to have such coverage. The fact that you are paid cash, should not hurt you, but the fact that you may not have filed tax returns hurts your credibility.
