I need information as to which forms to use for my cleaning business?

I have a new cleaning business. How should I word a form that my staff would sign pertaining to staff stealing clients and potential leads for cleaning. I am going out and bidding jobs, sending cleaning staff out to do the work ( and paying them). I need to be secure that I am building my new business and not allowing my staff to take my business. Lets say that a client really likes one of my staff and would like her to work for him, instead of my business.

I am aware of the non-disclosure and the confidentiality forms on legalforms.com but I am not sure which one to use or how to revise it to fit my business needs. I also need some kind of penalty if my staff steals clients. please advise me.

You are seeking a non-competition agreement, which must be written carefully in order to be enforceable. You are entitled to protect your trade secrets, and your customer list is one of those secrets. I doubt that you could fill out any form correctly, even if the form was a good form (which is questionable).
