If you fall behind on payments but are working with that company to make payments and then all of a sudden they refuse your payment is it considered paid off once they refuse that payment?

Absolutely NOT. It means that they are no longer working with you, and they will probably seek to collect the full amount of the debt in court. You should see an attorney in your area who practices debt and collections law (and possibly bankruptcy law) for a consultation on your case.
