Legal Question in Credit and Debt Law in Arizona

attorneys for a citi bank credit card company recently sent a "Request For Admissions" to fill out, are there any "sample forms" or examples on how to fill these things out, they are going into such legal details about things that make no sense to me.

There should be an area under each Request which allows you to enter your response of Admit or Deny. If you do not have enough room, then you can either retype the document or attach additional pages to your response.
