DAR - Notice of Disclosure and Request for Disclosure. what does that mean?

Arizona rules of procedure for court require that both parties provide "disclosure" of the information they intend to use at trial. A simplified explanation: "Notice of Disclosure" is what they are disclosing to you that they may use at trial, and request for disclosure is their request for yours. There are lots of rules and procedures- this is just a basic explanation of what you are looking at.
