Abandonment-Termination of Parental Rights- I NEED Help!!!!

I would like to know how to go about filing to have my ex-husband's parental right severed. Also I need to know which forms I need to file with the courts. And I would like to know if my children can still receive child support from him if his parental rights have been terminated. I have sole custody of the children and we live in Arizona. He has reasonable visitation and lives in Texas. The children have not seen their father in almost a year. He has had phone contact with them though.

As for termination of his parental rights, the court will not do that unless he desires it and there is some basis to do it. The fact that dad might not follow through with his parenting time has nothing to do with parental right elmination. However, if you desire, and there is a basis to do so, you might consider filing for a modification of parenting time (visitation)and support since he is currently getting credit for a certain amount of days and he is not fufilling his obligation.

As for support, if his parental rights are terminated, he no longer has an obligation to support his kids unless he agreed to do that in writing.
