adding a name

what do you have to do to add an additional name to a minor,s current legal name.ie:(John E. Jones Smith) Smith being the added name. Do both parents have to be in agreement? Thank you

Thank you for your inquiry.

Changing a minor's name is done in the civil court unless it is in the context of a paternity or adoption case. If the parents are in agreement, the change occurs almost immediately, with little difficulty. If either parent does not agree, the "changing" parent has to be able to show the court that the name change is in the child's best interests.
