Amended Birth Certificate

I changed my name in the 70's and have used it since. There was no real paperwork then, but did change Soc.Sec.# etc. Have used name consistantly. Now need an Amended Birth Cert. for a passport. Do I need to go to court for this or what??? My current SS card shows the name I have used & still do. Since it has been over 35 years what are my options?? Thank you.

In order to update your birth certificate, you'll need a court order legally changing your name to the one you've been using for the past several decades. The process is fairly straightforward - an application to the court, a filing fee, consent of your spouse, potentially notice to the parent of any minor children you have, and then a short hearing to finalize the name change. After the hearing, you'll receive a certified copy of the order, and you can submit that to the appropriate Vital Records department to have your birth certificate corrected.
