Legal Question in Family Law in Arizona

Name Change

I am trying to find out how to have my husband's name legally changed and if I need an attorney to do this. He was born and given one name and that name is on his birth certificate. However, at 4 years of age, his mother started calling him something else so, the ''nickname'' is on all of legal documents.
