Legal Question in Family Law in Arizona

Name change for minor child

My daughter's birth certificate has my maiden name for her last name. Her biologocal father is listed on her brith certificate but she does not have his last name. I am getting married and was wondering if I need his permission to change her last name from my maiden name to my married name

Re: Name change for minor child

In order to legally change it, you must seek a Court Order. It is generally a very easy process. Dad must be notified of the proceeding, but does not necessarily need to consent.

We can absolutely help.

I can explain the procedures better by phone or in person. As you can imagine, there is too much to cover via e'mail. We offer free 1/2 hour consultations, in which we can discuss the matter in more detail.

Please call me directly to discuss the specifics of your case, or contact my assistant Alicia or my paralegal Jennifer to schedule a free 1/2 hour consultation.
